Mindful Steps Counseling Services is looking for a licensed Master Social Worker with a passion for enhancing the lives of seniors. This full-time position, primarily remote, provides a unique opportunity to directly impact the quality of life for elderly clients and their families. Work Environment\n

This role allows for a flexible work setting with the majority of your time spent remote while occasionally visiting our hospice center in Pueblo, Colorado. Our work schedule is tailored to meet client needs with a standard work week from Monday to Friday, 8:00 AM to 4:30 PM, providing essential flexibility for client interactions. You will also occasionally need to accommodate evening or weekend hours and be available on-call to support urgent situations as they arise.

Key Responsibilities\n

• Perform comprehensive assessments to evaluate seniors' emotional, mental, and social needs, ensuring a detailed understanding of their circumstances.

• Collaborate closely with clients and their families to develop individualized care plans that reflect their unique goals, preferences, and values.
\n
• Facilitate counseling sessions to provide emotional support, helping clients navigate the complexities of aging, illness, and life transitions.

• Advocate for the rights and needs of your clients, ensuring they receive access to the services and resources available in the community.

• Work in partnership with an interdisciplinary team of professionals, including nurses, caregivers, and healthcare providers, to coordinate comprehensive care.

• Conduct regular visits to monitor client progress and living conditions, addressing concerns to promote safety and quality of life.
\n
• Help mediate conflicts among family members regarding care decisions and support effective communication within the family unit.
\n
• Educate clients and families on community resources and support groups, promoting social engagement and overall well-being.

• Maintain meticulous records of client interactions, assessments, and care plans, ensuring compliance with all relevant policies and ethical standards.

• Stay current on social work practices, policies, and regulations to enhance your practice and contribute to organizational excellence.

Patient Population / Client Focus\n

The primary focus of this role is working with seniors who receive services at our hospice center. This population often faces unique emotional and psychological challenges, such as coping with end-of-life care, dealing with loss, and managing chronic health conditions. You will be essential in crafting a supportive environment where they and their families feel understood and valued.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
\n
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of professional experience in social work, ideally within a community center or geriatric care setting.
\n
• In-depth knowledge of social work principles as they relate to aging and elderly care.
\n
• Exceptional assessment and counseling skills, with a strong emphasis on problem-solving.
\n
• Compassionate, empathetic demeanor with the ability to build genuine rapport with seniors and their families.
\n
• Strong verbal and written communication skills, facilitating effective teamwork in an interdisciplinary environment.

• Proficiency in using electronic health record (EHR) systems and other digital tools for documentation and communication.

Preferred Experience\n

• Experience working in hospice care or end-of-life services.

• Familiarity with community resources and programs catering to senior citizens.

• Background in family counseling or mediation.

Compensation and Benefits\n

The compensation for this role ranges from $60,000 to $100,000 per year, commensurate with experience and qualifications.
