This is a fully remote position, allowing you to provide services from the comfort of your home while ensuring accessibility for your clients in various settings, including their homes or community centers. Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the psychosocial needs of seniors, focusing on emotional, mental, and social support systems.

• Collaborate closely with clients and their families to create and implement personalized care plans tailored to their individual goals and preferences.

• Provide counseling and emotional support during life transitions such as relocation to assisted living or coping with health challenges.
\n
• Act as an advocate for clients, ensuring their rights are upheld and facilitating access to essential community resources.
\n
• Work as a part of interdisciplinary teams, communicating effectively with nurses, healthcare providers, and caregivers to optimize client care.

• Conduct regular home visits to monitor clients’ well-being, identify any safety concerns, and ensure their living conditions are supportive of their needs.

• Resolve conflicts among family members and facilitate discussions to enhance cooperation in care decisions.

• Educate clients and families about available programs and support groups that promote social engagement and improve overall well-being.

• Maintain detailed and accurate documentation of assessments, treatment plans, progress notes, and all relevant interactions to ensure continuity of care.

• Stay informed on the latest social work policies and regulations, ensuring adherence to ethical standards and best practices within the field.

This role focuses on the geriatric population, specifically targeting the unique challenges faced by seniors. Qualifications\n

• Bachelor's degree in Social Work or a related field from an accredited institution.

• Current and valid Social Worker license in the state of Washington (LICSW preferred).

• A minimum of two years of experience in a social work capacity, ideally with a focus on geriatric services or community health.

• Strong understanding of social work principles, aging-related challenges, and the spectrum of available community services.

• Exemplary assessment and counseling skills to engage effectively with clients and their families.

• Exceptional communication capabilities that enable seamless collaboration with multidisciplinary teams.

Compensation and Benefits\n
