Life Stage Counseling is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join our dedicated team in Silverthorne, Colorado. This is a full-time position that offers a unique opportunity to work in a remote setting within a school environment, providing essential support to seniors and enhancing their quality of life. Work Environment\n

This position is primarily remote, allowing for flexibility in work hours. You will be working with a vibrant interdisciplinary team that includes healthcare professionals, caregivers, and educators who are dedicated to optimizing the health and happiness of our senior clients. The working hours are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend assignments as needed to accommodate client requirements.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of clients, evaluating emotional, social, and mental health needs, as well as their living environments.

• Collaborate with clients and their families to develop and implement personalized care plans that address specific needs and goals.
\n
• Provide counseling services that offer emotional support and guidance through life transitions, illness, or bereavement.
\n
• Advocate for clients by connecting them with community resources, ensuring they have access to necessary support systems.
\n
• Work in tandem with a multidisciplinary team, including healthcare providers and educational staff, to coordinate services and enhance the overall care experience.
\n
• Schedule regular follow-up visits to monitor clients’ progress and reassess their changing needs.
\n
• Facilitate communication among family members to promote understanding and collaboration in caregiving.
\n
• Educate clients about available programs and resources that encourage social engagement and enrich their lives.

• Maintain documentation of client observations, care assessments, and interactions in compliance with organizational standards and regulations.

• Continuously update knowledge on social work practices and evolving community resources to improve service delivery.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience in social work, particularly in community counseling or a geriatric setting.

• Comprehensive knowledge of social work practices, aging-related challenges, and available social services.

• Exceptional interpersonal, assessment, and counseling skills, with a proven ability to establish rapport with seniors and their families.

• Strong organizational skills with proficiency in electronic health record systems for maintaining client documentation.

• A commitment to respect, dignity, and advocacy for client rights.

Compensation and Benefits\n
