This role will be primarily remote, allowing you the flexibility to manage your schedule while ensuring that you can provide continuous care to your clients. Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors.

• Collaborate with clients and their families to develop personalized care plans that align with their specific goals and preferences.

• Provide counseling and emotional support, helping seniors and their families navigate life transitions such as relocation to assisted living or coping with health changes.

• Advocate for clients’ rights and ensure their access to community resources, services, and support needed for their overall well-being.

• Work collaboratively with an interdisciplinary team including nurses, caregivers, and healthcare providers to enhance patient care and coordinate services effectively.

• Perform regular visits to monitor clients’ well-being and living conditions, ensuring safety and addressing any immediate concerns.

• Facilitate communication and conflict resolution among family members involved in the senior client’s care process.
\n
• Educate clients and their families about community resources, support groups, and programs that promote social engagement and enhance quality of life.

• Maintain accurate documentation related to client assessments, care plans, progress notes, and any significant interactions in compliance with regulatory standards.

• Continuously stay updated on best practices in social work, adhering to ethical standards and policies relevant to aging populations.

Patient Population / Client Focus\n

The primary focus of this role is the senior population within the Hialeah community. You will support elderly clients facing various challenges, including mental health issues, social isolation, and difficulty accessing essential health services. You will also provide comfort and guidance to families navigating the complexities of caring for aging loved ones.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience in social work, preferably in a community health or geriatric setting.

• In-depth knowledge of social work principles, issues related to aging, and the landscape of available community resources.

• Exceptional assessment, counseling, and problem-solving skills.
\n
• Ability to empathize and build rapport with seniors and their families, demonstrating compassion in every interaction.

• Strong communication and interpersonal skills, essential for successful teamwork and collaboration.

• Proficiency in electronic health record (EHR) systems and familiarity with relevant software applications.

Preferred Experience\n

This position offers a competitive salary ranging from $60,000 to $100,000 annually, commensurate with experience and qualifications.
