Key Responsibilities\n

  • Conduct comprehensive evaluations and psychosocial assessments to identify the needs and concerns of clients and their families.

Your client focus will primarily include elderly individuals receiving hospice services, along with their families who may require additional support during the emotional and psychological challenges of end-of-life transitions. You will work with a diverse population, addressing various psychosocial issues and ensuring that each client receives tailored care that respects their unique circumstances and preferences.

  • A Master's degree in Social Work (MSW) from an accredited program is mandatory.

  • Possession of a valid LCSW licensure in the state of Texas is required for this role.

  • Exceptional verbal and written communication skills are essential, enabling effective interactions with clients, families, and colleagues.

  • Demonstrated expertise in risk assessment, crisis intervention, and diagnosis of mental health conditions relevant to the geriatric population.

  • Strong organizational skills and the ability to manage multiple cases effectively.
